Diagnostic protocol from the Annals of Internal Medicine

Holmes et al published guidelines in 1988 in the Annals of Internal Medicine. The matter was reviewed in 1994 (Fukuda et al Ann Int Med 1994; see appendix II in CFS Handbook) when an international panel suggested the following diagnostic guidelines:
This was updated by the Canadian Criteria,

A thorough medical history, physical examination, mental status, and laboratory tests must be conducted to identify underlying conditions that require treatment. Diagnosis cannot be made without such an evaluation. A patient must satisfy these criteria, to receive a diagnosis of CFS.
1. Clinically evaluated, unexplained persistent or relapsing chronic fatigue of 6 months or longer duration that is of new or definite onset, not lifelong, with other known medical conditions excluded by clinical diagnosis. It is not the result of ongoing exertion, not substantially alleviated by rest, and results in substantial reduction in previous levels of occupational, educational and social activities.

2. The concurrent occurrence of four or more of these symptoms:
substantial impairment in short-term memory or concentration;
sore throat;
tender lymph nodes;
muscle pain;
multi-joint pain without swelling or redness;
headaches of a new type, pattern, or severity;
unrefreshing sleep;
post-exertional malaise lasting more than 24 hours.

These symptoms must have persisted or recurred during six or more consecutive months of illness and must not have predated the fatigue

A comprehensive medical history and careful physical examination is required to make the diagnosis

Comprehensive blood tests (SMAC )/ CBC

Thyroid panel, T3,T4 and TSH

...Thyroid peroxidase

SERUM TESTS for :

...Viral titers of HHV6 EBV, CMV

...serum amino acids... including carnitine,glutathione, cystine,argenine,methionine

...Lymes Disease --- Western Blot, Eliza and PCR

...Mycoplasma pneum., ...Clamydia pneum., ...Candida

...Babesia , ...Erlichia

EKG , cardiac output Impedance Cardiography( this is an upgraded tilt table. Dr Enlander is one of the few physicians with this new Cardiac Output equipment), Spirometry

Cancer screening tests Ca125 (female Ovarian) ; PSA/ PAP (male prostate)
